National patient survey for Level 1/Surgical Centres:
You will find the national patient survey for Level 1 Surgical Centres in England at www.chdpatientsurvey.co.uk . There are different surveys designed for inpatient and outpatient services, for three groups: children (under 14s); their parents/carers; and teenagers and adults (over 14s).
