When you are a young person who has been diagnosed with a cardiac condition in childhood, you may not know what to expect when you move from children's to adult services (who will you see and what will happen next).

In this section you will find more information about the transition pathway. A pathway is a term we use to describe the different steps on a healthcare journey and transition refers to the move from children's to adult services.

Our specialist hospital teams have designed this interactive pathway document to show you the steps that you may take on your journey.

Please also access the Babies and Children, and Adult pathway areas of the website for more information on the more general pathways you may experience as part of your healthcare journey. (If you are currently still with children's services, please use Babies and Children, if you want information on the pathways in adult hospitals, please use the Adult section.)
